This Certificate of Coverage (COC) is issued to Medicaid Program recipients who have enrolled in Procare Health Plan, Inc. The terms and conditions of this certificate are governed by the compiled laws of the State of Michigan and the Medicaid benefits thatProcare Helath Plan, Inc is required to provide pursuant to its Medicaid Agreement with the State of Michigan.
