SWEET REWARDS SELF-MANAGEMENT DIABETIC PROGRAM
Pro Care takes the disease, diabetes seriously. With this in mind, Pro Care has developed a diabetic program that involves a supportive health care environment. This program is dedicated to
the sole purpose of providing diabetic members the assistance and encouragement they need in sustaining a healthy lifestyle. In conjunction with area hospitals and outside health care agencies,
the Pro Care Diabetic Program instructs members toward attaining the goal of diabetic self-management.
MOMMY & ME MATERNITY INFANT HEALTH PROGRAM (MIHP)
Pro Care’s MOMMY & ME program facilitates and ensures maternal and infant support services to enrolled high-risk pregnant women and newborns. These specialized preventative services are
offered to help promote the reduction of infant mortality and morbidity. A multidisciplinary team of health professionals (through contractual arrangements) concentrates on providing medical
services, social services, and nutrition and health education.
I CAN QUIT SMOKING CESSATION PROGRAM
Pro Care is committed to assisting those enrolled members who wish to stop smoking. In an effort to facilitate this positive change in behavior, Pro Care has teamed up with American Cancer Society, a known
leader in health behavior change to offer the smoking cessation program. The I Can Quit smoking cessation program has been proven to be the difference between success and failure.
MOMMY & ME
MATERNITY INFANT HEALTH PROGRAM
Pro Care’s MOMMY & ME program facilitates and ensures maternal and infant support services to enrolled high-risk pregnant women and newborns. These specialized preventative services are offered
to help promote the reduction of infant mortality and morbidity. A multidisciplinary team of health professionals (through contractual arrangements) concentrates on providing medical services,
social services, and nutrition and health education.
This program supplements routine prenatal care and is intended to improve prenatal outcomes. Members who are likely to have serious psychosocial or nutritional conditions receive the following
services:
- Psychosocial and nutritional assessments
- Plan of care development
- Professional interventions
- Care coordination
- Referrals to specialists and linkage to community resources
- Childbirth education (including the importance of receiving routine prenatal care, Well Child visits and immunizations, as well as other necessary health services)
- Transportation as needed for health, substance abuse treatment, support services and /or pregnancy –related appointments
- I Can Quit an Individualized smoking cessation program, (if requested or applicable)
- Sweet Rewards a diabetic educational program, (if applicable)
Primary care providers, obstetricians / gynecologists, care providers and Pro Care administration through information obtained from encounter data outside the MOMMY & ME program
can identify, screen and initiate referrals.
A referral can be initiated if the mother has at least one decisive factor of the following:
- Homeless or dangerous living/home situation
- Negative or ambivalent feelings about the pregnancy
- Under 18 years of age and no family support
- Requires assistance to care for herself and her infant
- Is cognitively, emotionally, or mentally impaired
- Has nutritional problems
- Abuse of alcohol or drugs
- Tobacco use
- Needs transportation assistance to keep medical appointments
- Requires childbirth education
MATERNAL INFANT SUPPORT PROGRAM
Pro Care’s Infant Support Services also expands the following services to enrolled members with young children. Home visits for support, teaching and referral are available to help promote healthy
parents and children. If necessary Pro Care will initiate the following:
- Public Health Nurse home visits
- Social Work interview / home visits
- Dietician interview / home visits
Travel reimbursement for doctor office visits, WIC, counseling, and parenting classes.
Because the focus of this program is prevention and control of child health problems through education, monitoring and referral to appropriate support agencies, a referral can be initiated if the
infant has at least one decisive factor:
- Poor access to care
- Teenage Mother
- Difficulty with breast feeding at 6 months
- Evidence of child abuse
- Special health care needs
- Need for immunizations
- Elevated blood lead levels
- Low birth weight
- Signs of poor nutritional intake: dehydration; obesity
- Perinatally acquired HIV
- Frequent hospitalizations
- Homeless or dangerous living / home situation
